PERKIOMEN VALLEY SCHOOL DISTRICT PERMISSION TO PARTICIPATE IN SKI CLUB

The Perkiomen Valley High School, Perkiomen Valley Middle School East, and Perkiomen Valley Middle School West recognize the Ski Club as an approved extra-curricular student activity. All Perkiomen Valley High School, Perkiomen Valley Middle School East and Middle School West students are eligible to participate in Ski Club, subject to all terms and restrictions set forth in applicable Board policy and administrative guidelines and applicable limitations on the number of participants.

In order for a student to participate, this form must be completed and returned to the applicable Ski Club faculty advisor no later than the day before the trip. No child will be allowed to participate without a completed permission form.
