PERKIOMEN VALLEY SCHOOL DISTRICT PERMISSION TO PARTICIPATE IN SKI CLUB

The Perkiomen Valley High School, Perkiomen Valley Middle School East, and Perkiomen Valley Middle School West recognize the Ski Club as an approved extra-curricular student activity. All Perkiomen Valley High School, Perkiomen Valley Middle School East and Middle School West students are eligible to participate in Ski Club, subject to all terms and restrictions set forth in applicable Board policy and administrative guidelines and applicable limitations on the number of participants.

In order for a student to participate, this form must be completed and returned to the applicable Ski Club faculty advisorno later than the day before the trip. No child will be allowed to participate without a completed permission form.

We/I acknowledge that we/I have read this agreement, understand it and willingly, voluntarily and without reservation agree to its terms and conditions.

Parent Permission and Release of Liability

By the very nature of the activity of skiing/snowboarding, participants are at risk of physical injury. No matter how careful the participant and supervising adults are, no matter how many precautions are taken, the risk cannot be eliminated. The risk of injury includes minor injuries such as broken bones, dislocations and muscle strains, but also includes catastrophic injuries such as permanent paralysis or even death.

Participation in Ski Club activities are voluntary and offered purely for the extracurricular benefit of the students. No pressure, direct or indirect, has been or is being exerted to encourage participation in these activities.

do hereby acknowledge that we/I have been fully advised, cautioned and warned by the proper school personnel of the Perkiomen Valley School District that our/my child named above may suffer minor and/or serious injury by participating in Ski Club activities. Notwithstanding such warnings, and with full knowledge and understanding of the risk to our/my child named above, we/I give our consent for our/my child to participate in the Ski Club and all related activities and assume the risk of injury to our/my child resulting from his/her participation therein.

We/I acknowledge that the Perkiomen Valley School District, its directors, administrators, employees and volunteers make no warranties or guarantees regarding the degree of supervision of individual or group conduct during Ski Club activities and the School District, its directors, administrators, employees and volunteers assume no responsibility for such conduct and/or any injuries resulting therefrom.

We/I specifically release and waive any and all claims, causes of action and rights whatsoever that we now or may ever have against the District, its directors, administrators, employees and volunteers as a result of our/my child’s participation in Ski Club activities.

Medical Emergency Authorization and Health Information

In case of sudden illness or an accident to the above named participant requiring immediate treatment or surgery while he/she is a participant in any Ski Club activity, I authorize the District, its employees and/or volunteers to take such action as deemed appropriate to protect the health and physical well-being of the above participant. This authority extends to any physician(s) and/or surgeon(s) selected by school personnel to perform medical and/or surgical procedures including examinations and tests necessary to preserve the health and physical well-being of the above named participant. All efforts will be made to contact the parent(s) or guardian(s) in case of emergency.

The following information is provided as an aid to school personnel in dealing with the well-being of the participant. The participant has the following health conditions: (include allergies, disabilities, diabetes, pregnancy, asthma, medication needed, etc.) I certify that my child is in good physical condition and able to participate in Ski Club.

Behavior Agreement

All participants in the Ski Club will be expected to comply with the following code of conduct. Failure to adhere to the code of conduct shall constitute grounds for disallowance of further participation in the Ski Club.

  1. The health, safety, and welfare of others must be respected at all times.
  2. Appropriate language and behavior are expected at all times. Profanity, foul or abusive language, inflammatory statements, derogatory comments, or physical altercations toward any group or individual are not permitted.
  3. Participants are responsible for following the instructions of all supervising school personnel, including volunteers.
  4. All behavior or language of a sexual nature is inappropriate and unacceptable. Dignified and respectable behavior is expected at all times.
  5. Compliance with all rules and regulations of the Perkiomen Valley School District, as well as the applicable ski facilities, is required.

Media Policy and Release

The School District routinely promotes activities through various media. This includes, but is not limited to newsletters, newspapers, brochures and displays. In doing so, the names and photos of members and participants may be included. Youth in photos will not be identified by name(s).

I have read the Behavior Agreement and Media Policy and Release above and discussed it with my son/daughter. I understand and agree to the conditions set forth.

Skier Responsibilities and Safety Tips

  1. DO NOT SKI ALONE! Use the buddy system at all times.
  2. SKI UNDER CONTROL and in the manner that you can stop to avoid other skiers or objects.
  3. Skiers approaching each other on opposite traverses shall pass to the right. When overtaking another skier, you must avoid the skier below you. (Uphill skier must yield.)
  4. You must stop on the side of a trail. Do not obstruct traffic.
  5. Do not delay or stop on the downhill side of a mogul.
  6. When entering a trail or starting downhill, yield to other skiers. Check uphill traffic before crossing a trail.
  7. When overtaking another skier, say “on your left” or “on your right”, so they are aware of your position.
  8. You must not enter closed trails.
  9. Respect skiers on beginner slopes. Be cautious of slower skiers.
  10. DO NOT SKI ABOVE YOUR LEVEL.
  11. Never jump from the chair lift. In the event of malfunction, the ski patrol will assist you.
  12. If you have a collision resulting in an injury to another skier, it is your responsibility to stay at the site of the collision until the ski patrol arrives. Cross your skies and stand them up, above the injured person to alert other skiers. Ask someone to report the injury to an employee, so that the ski patrol can be alerted.
  13. Do not cut into lift lines.
  14. Do not wear loose clothing or flowing scarves.
  15. Go inside when you are cold.
  16. Rest when you are tired. Fatigue leads to falls.
  17. As a first time skier/rider I will take a lesson before skiing/riding.
